Instructions: Complete 1 form per congregation at https://forms.gle/m7GjCCBtNVaSFLAQ9 Identify 1 main adult contact per congregational group. Put the number of youth & adults playing paintball. There is another spot to indicate additional chaperones attending who may choose not to play. Bring exact cash or check made payable to Immanuel Youth the day of the event. Remember, all minors must have a parent or guardian sign the online waiver. Every group must provide appropriate chaperones for their group. If you are signing up as an individual, you must still have a chaperone if under 18.
